In order to start the process of me working with you and your canine companion please complete and return a New Client Form and I will respond within 48hrs. I may ask you to complete a Behavioural History Form when I respond if I require more information.
IMPORTANT: Please also read my Policies and Procedures and Service Contract as sending me the New Client Form means you have read and agreed with these terms and conditions. Please complete, sign the pdf forms on screen and return the completed forms to karen@woofsofbournemouth.co.uk . You can either save the forms to your desktop and add as attachments to the email, or send forms one at a time through Acrobat by clicking the send icon (top right) and selecting 'email this page'. These important forms are also listed below for your convenience.
